Adoption Application

Please make sure you have read through the adoption information before applying. Please fill out the adoption application below.  Make sure you have all of the information needed, including Vetrinarian contact information and reference contact information.

If you are unable to complete the online application, please download and fill out the adoption application and email it to adoptapp5543@yahoo.com.

Application Processing Steps

Rent/Lease? - We need your landlord letter first!

Own a Condo or Townhome? - We need your HOA letter first!

1. Vet Check - Please immediately authorize your vet to release information to AGSDR.
2.  Reference Check - Please notify your references that AGSDR will be calling!
3.  Home Visit – Yes, someone will visit you, this is a step for ALL applicants.
4.  Dog Visits – If you are approved, your coordinator will put you in touch with foster families.
5.  Contract – Adoption Contracts will be executed for all adoptions.  Read before you sign!
6.  Fee – The fee to adopt is a minimum adoption fee of
   • Puppies less than 1 year: $400
   • Adults 1 year  - 6 years: $350
   • Seniors 7 years or older:  $250
   • Note: Additional donations are welcomed.
